Patient: Add problem status options (in progress, resolved, etc.) to patient's problem list

Ability to determine what the status of a problem is and modify it from the patient's Problems tab.

Problems should be able to be made discontinued (show start-end dates), made chronic, intermittent, waxing-waning, resolved etc. And should automatically show up on future consults if still active.

    Ability to edit the problem list in the consultation would be wonderful as well (move a problem from active to resolved, etc).
