Forms: Add default headers and footers on forms (practice defined, optional)

There should be a way to have the forms have a dedicated header or footer that will print on the bottom of every page.

College of Veterinarians of Ontario requires that each page of medical record has a header and footer.
Header should include Patient name, ID at the very least as well as Client name and ID. The footer should include the date of the record (not the date the record was printed) and the page number of the document.
