Add "DUE DATE" to 'vaccine certificate with reminders' form right above "Sincerely" and to right of "reminders for pet"

This form is frequently given to boarding facilities, groomers, HOAs to show current vaccine status. To make it less ambiguous for those people, adding a DUE DATE caption above the actual due dates would make the reminder aspect much clearer.
