Reminders: Add Appointment Reminder to drop down in reminders, and make "Default Reminder" editable.

If we want to put in a specific reminder that is not part of the drop down, the default is not editable. For example, if you wanted to be able to enter a reminder to re-check an abnormal thyroid result in 3 weeks.

    You can currently add reminders to the Reminder drop-down list in Administration > Reminders. See Add a reminder for more information.
