Patients: Add number to Problem tab to show that there are problems listed like is added to Files tab when files are added

When you look at a consult you can see Problems, Vitals, Files, etc across the top. If files have been added there is a number in parentheses. Do this for problems too so we will know without opening the tab if problems have been listed for this patient.
