Consultations: Add vital signs to a consultation after it's been closed

At present you can add vital signs to the vitals tab for the date it was in, but it will not add it into the consultation and if the consultation is closed there is no way to go back and add in the vital signs to that consult. We'd like to be able to update this after the consultation is closed.

    This is also a problem because when you 'print' records from the consultation tab, if the weight was added to the vitals tab but not a consultation then it doesn't show up in the records anywhere that are being sent.
