Custom Extra Fields with Drop-Down Options (Particularly for Preferred Providers)

I would like to make an extra field in the client page that gives the drop down option of choosing their preferred provider. It would also be great if it could have an attached alert when booking an appointment with a doctor OTHER THAN their preferred provider.
