Reports: Compliance reminder to include client and patient information

Clinic would like the report to contain the following information:

The patient has a reminder with a due date
The patient has no consultations .
The type of reminder has not been deactivated.
The patient reminder itself has not been deactivated.
The patient and the client both have an active status.
