Appointment: Track duration of each stage of visit (check-in, assigned to room, tech/dr completed to check out)

Buttons to capture time stamps and user id for stages of a visit. When checked in, when patient first seen, when appointment finished. Most of this information is already captured. Lastly reporting to get average times for each stage by provider and reason for visit. This would help clinics improve efficiency.
