Client / Patients: Create system to manage customer loyalty program (ex: reward points, rebate, discount, etc.)

We would like to be able to reward clients/patients with points and then later use those points to redeem as a discount or free goods. A easy way to do this would be with a special action under the item that adds X to a field in the patient file so each item can give x points and they can remove the points as they use them under the patient file.
